Please read through the volunteer checklist below:
- If you are a first time volunteer, please fill out this form online.
- Everyone MUST have signed the HIPAA Form before coming onsite. Please see the link below: https://hipaa.jotform.com/210443923290046
If you already signed the online HIPAA Form in the past, you do NOT need to sign again. - If you are a vaccinator you must have an active license as proof of your ability to vaccinate. Please send to covid@skippackpharmacy.com
- Volunteer t-shirts and lanyards will be available at the volunteer check-in stations.
